Thank you for your interest in becoming a valued customer! To set up your account, please complete our New Customer Application Packet and send it to accounting@shadenlight.com.

How to Apply:

  1. Download the New Customer Application Packet (PDF)
  2. Fill out the form completely, including:
    • Company and contact information
    • Billing and shipping details
    • Trade references (Indicate the vendors from whom you purchase)
    • Bank information
    • Payment details
    • Authorized signature
  3. For tax-exempt Florida businesses, please attach a copy of your DR-13 Resale Certificate.
