Thank you for your interest in becoming a valued Shade & Light Window Fashions customer! To set up your account, please complete our New Customer Application Packet and send it to accounting@shadenlight.com.
How to Apply:
- Download the New Customer Application Packet (PDF)
- Fill out the form completely, including:
- Company and contact information
- Billing and shipping details
- Trade references (Indicate the vendors from whom you purchase)
- Bank information
- Payment details
- Authorized signature
Please note: This form may be completed digitally; however, it must be printed and signed. After signing, please scan or photograph the completed form before submitting.
For tax-exempt Florida businesses, please include a copy of your DR-13 Resale Certificate.
